Application value of magnetic resonance imaging in patients with apical hypertrophy cardiomyopathy

Abstract:
Objective To evaluate the clinical value of cardiac magnetic resonance (CMR) in the diagnosis of apical hypertrophic cardiomyopathy (AHCM). Methods Nine patients with AHCM from 2017 to 2018 in our hospital were successfully underwent magnetic resonance scanning. Myocardial thickness, ventricular atrial volume, left ventricular ejection fraction were measured. Myocardial involvement segments, the characteristics of myocardial first perfusion and delayed enhancement were evaluated. Results CMR showed apical hypertrophic in all the patients. These patients were divided into two groups: pure AHCH and mixed AHCH. Among them, 6 patients were pure AHCM, 3 patients were mixed AHCM. 3 patients had apical aneurysm. All patients underwent myocardial contrast enhancement scanning, 8 patients presented late gadolinium enhancement (LGE) which showed plaques and focal enhancement. 5 pure AHCH’s LGE were located in the apex. 3 mixed AHCH’s LGE were involved the central left ventricle and even the papillary muscles. Conclusions CMR is best diagnostic modality for AHCM. LGE can characterize areas of myocardial fibrosis, which plays an important role in the natural history of AHCM.
Advantages of dual energy CT direct enhancement in dual lower limb vein imaging

Abstract:
Objective To explore the application of rapid double-energy CT scan in the direct enhancement of bilateral lower limb veins (DE-DSCTV), and the advantages of the direct enhancement method relative to the indirect enhancement method. Methods Thirty patients with lower limb venous disease were selected. The dual-energy CT enhanced scan of bilateral lower extremities was performed by improving the injection mode of high-pressure syringe. 3D vascular images were obtained by post-processing and compared with the previous indirect enhancement CTV image of the same patient. Judging the feasibility of the direct method to enhance the venography of bilateral lower extremities, and whether the direct enhancement method DE-DSCTV is more advantageous. Results All 30 patients had higher contrast, clearer and more accurate images, which greatly improved the detection rate of lesions. Conclusion Bilateral lower extremities venous double energy CT direct enhanced CTV is a low dose and low contrast medium examination. The direct enhanced images of both lower extremities can be obtained simultaneously by one scan. Compared with the indirect enhancement method, the image obtained by the direct enhancement method has a higher contrast, which is more favorable for the detection of the focus, and the imaging effect is satisfactory.

Abstract:
Objective To investigate the role of MEG combined with Wada test in the assessment of language and memory function in epilepsy patients before surgery. Methods Twenty-six cases of epilepsy patients, preoperative localization of epileptogenic focus and neuropsychological assessment; the combination of MEG and Wada test, to determine the language memory, dominant hemisphere, clear language function area and memory function. IQ, Memory, language function of patients were compared before and 1 year after surgery. Results Twenty-six patients with epilepsy were checked by MEG and Wada test the process smoothly, can lateralization of language and memory of the dominant hemisphere, 20 cases of left hemisphere language dominance, 6 cases were located on the left side on the right side of the. 14 memory of the dominant hemisphere, 8 cases located in the right hemisphere, 4 cases of bilateral.MEG can accurately locate the position of the. WADA language function can be evaluated in patients with memory comparison of memory function. The IQ, memory and language function were measured before and after 1 year. Verbal IQ and total IQ increased (P<0.05), and no significant change in memory quotient and operation IQ (P>0.05). The dominant hemisphere and non dominant hemisphere, memory score was significant (P<0.05). Conclusion Wada test recovered well after operation, without serious language and memory impairment. MEG combined with Wada test can accurately locate the lateral language, memory the dominant hemisphere, and understand the memory function before operation. It is of great significance for guiding the resection range and avoiding postoperative complications.
Value of traditional ultrasound, color doppler and ultrasound elastography for prediction axillary lymph node metastasis of breast cancer

Abstract:
Objective To discuss the value of traditional ultrasound, color doppler and ultrasound elastography for diagnosing axillary lymph node (LN) metastasis in breast cancer. Methods Axillary LNs in healthy probands (n=56) and metastatic LNs in breast cancer patients (n=26) were examined by suing traditional ultrasound, color doppler and ultrasound elastography. Short radius, long axis, S/L, cortical thickness, blood flow characteristics and elastic characteristics of all LNs were detected, and then these indicators were compared between the two groups. Receiver operating characteristic(ROC)curve of all above indicators were drawed, and AUC, threshold value, sensitivity (SE) and specificity (SP) were calculated. Moreover, the relationship between cortical thickness and the hardness was studied. Results The short radius, S/L, and cortex thickness of metastatic LNs were all significantly larger than that of non-metastatic LNs (P=0.000). The blood types in metastatic LNs were almost III and IV types. UE displayed that the scores of most metastatic LNs was 3-4, but the scores of most non-metastatic LNs was 1-2 (P<0 01="" the="" se="" of="" short="" radius="" cortical="" thickness="" s="" l="" doppler="" ultrasound="" and="" sonoelastography="" was="" revealed="" to="" be="" 84="" 6="" 84="" 6="" 84="" 6="" 76="" 9="" and="" 84="" 6="" and="" the="" sp="" was="" 73="" 9="" 97="" 8="" 84="" 8="" 95="" 7="" and="" 97="" 8="" respectively="" the="" highest="" se="" and="" sp="" was="" achieved="" by="" the="" b-mode="" cortex="">2.25 mm) and elastographic features (3 score). The cortex thickness had a strong correlation with hardness of LN (r=0.899, P<0.00). Conclusion Cortex thickness and UE of LNs are both feasible method to identify the metastatic LNs from non-etastatic LNs.

Abstract:
Prenatal ultrasound plays an irreplaceable role in diagnosing fetal cardiovascular disease. With the increasing development of prenatal ultrasound medicine and ultrasound instruments, the testing methods for fetal cardiovascular function have been further improved, and changes in some ultrasound indicators can reflect changes in fetal heart, cardiac and cardiovascular function. This paper reviews the application of ultrasound index in evaluating fetal cardiovascular function and provides reference for further studies.

Abstract:
Objective To explore the imaging features and reasons of misdiagnosis in ovarian fibrothecoma. Methods The clinical and imaging data of 16 female patients with pathology confirmed ovarian fibrothecoma were analyzed by two radiologists. The patients were conducted of CT or MR imaging examinations. The location, shape, maximum diameter, signal intensity or density, and enhanced pattern of the lesions were depicted. The reasons of misdiagnosis were explored. Results The tumors involved unilateral ovaries in all 16 patients, with the average maximum diameter of 10.9 cm. All the lesions showed the inhomogeneous signal intensity or density. Twelve lesions exhibited solid or solid -predominant mixed mass, and 8 lesions were found hypointensity area of parenchymal part on T2 weighted images. With the injection of contrast to 15 cases, 13 lesions presented slightly to moderately enhancement. Increase of serum CA125, estrogen level, and pelvic effusion were found in 9, 2 and 2 patients, respectively. Of 16 cases, 3 cases were misdiagnosed for epithelial tumors, and 3 cases for subserosal fibroids of uterus or broad ligament fibroids. Conclusion The ovarian fibrothecomas exhibit certain characteristic imaging features, but they are prone to be misdiagnosed.

Abstract:
Left atrial/left atrial appendage (LA/LAA) thrombosis is the most serious complication of atrial fibrillation. It not only affects the cardiac output function, but also leads to important arterial embolization following the shedding of emboli with blood circulation to all parts of the body, especially to acute brain stroke. Cardiac magnetic resonance imaging (CMR) has received widespread attention as a new non-invasive technique. It can dynamically observe the motor function and hemodynamics of left atrial and left atrial appendage, and has a high detection rate of LA/LAA thrombosis. CMR is expected to replace TEE in the clinical application of diagnosing LA/LAA thrombosis.

Abstract:
Objective To analyze the experience of 22 patients with intraventricular tumor undergoing neuroendoscopic surgery. Methods A 1 cm in diameter cranial hole was drilled on the skull and operate in the space created by the large channel neuroendoscope with a transparent shell. Using a clamp type bipolar coagulation and scissors, the choroid plexus and blood vessels that were associated with the tumor were disconnected from the vessel. Finally, the tumor was completely removed. If the tumor size was too large, endoscopic ultrasound surgical aspirator was used to reduce the volume of the tumor and remove it. Results Twenty-two cases of ventricular tumors were completely resected. Conclusion The coordination of the nurses during the operation can help the surgeon perform the operation more smoothly. The key points for nurses to cooperate with the surgeon are: to set up the surgical position according to the location and size of the tumor; to accurately install and transmit various neuroendoscopic surgical instruments. Especially the replacement of different types of neuroendoscope, wipe endoscopic bipolar coagulation efficiently and the correct use of endoscopic ultrasonic surgical aspirator.

Abstract:
Objective To observe the blood supply artery of uterus and ovary in female pelvic organs by ultrasonic kinetics, and to provide the basis of hemodynamic for clinical diagnosis of good and malignant ovaran and uterine tumor. Methods From January 2017 to January 2018, 50 women with ovarian and uterine tumors were selected. All patients underwent surgery or laparoscopic specimens to confirm the pathological type. They were divided into four subgroups: group A was benign ovarian tumors; Group B is malignant ovarian tumors; Group C is uterine benign tumors; Group D is uterine malignant tumors, 25 cases in each group.High-frequency ultrasound was used to determine the morphological classification of blood flow under ultrasound, the resistance index and pulsation index of ovarian artery and uterine artery, and the specificity and sensitivity difference of ultrasound hemodynamics in the diagnosis of ovarian and uterine malignant lesions were analyzed by ROC curve. Results The ovarian and uterine benign tumors resistance index and pulsatility index were higher than malignant tumors (P<0.05). The internal signal and peripheral signal detection rate of ovarian and uterine benign tumors were significantly different from those of ovarian and uterine malignant tumor s (P<0.05). The sensitivity of Vaginal ultrasound Doppler diagnosis for ovarian cancer was 76.21%, and the specificity was 80.21%. The sensitivity of vaginal ultrasound Doppler diagnosis for uterine cancer was 73.36%, and the specificity was 76.72%. Conclusion Color Doppler ultrasound can achieve early screening for malignant ovarian cancer and malignant uterine cancer through hemodynamic parameters, which has important clinical value.

Abstract:
Objectives To explore whether C7 pelvic angle can evaluate the sagittal balance, and investigate the relationship between CPA and health related quality of life of patients with degenerative scoliosis (DS). Methods Data of DS patients who received surgical treatment in our hospital from February 2010 to January 2017 were retrospectively analyzed. A total of 67 DS patients were included (7 male, 60 females). Results The correlation between preoperative SVA, C7/SFD and sagittal plane balance parameters was weaker than CPA. At the last follow-up, the correlation between CPA and sagittal equilibrium parameters was stronger than that between SVA and C7/SFD. Preoperative SVA, C7/SFD and CPA were correlated with VAS, ODI and SRS-22 (P<0.05), and CPA was more correlated with them. The correlation between CPA and health related quality of life score was stronger than SVA at preoperative and final follow-up. Conclusion CPA can better reflect spinal-pelvic sagittal balance and health related quality of life, and the guiding significance of DS reconstructive surgery is more important.

Abstract:
Objective To investigate the clinical value of seminal vesicle endoscopy technique in the diagnosis and treatment of hemospermia. Methods Nineteen patients clinically diagnosed with refractory hemospermia were included in the study. Prostatic fluid culture and drug sensitivity test were performed before the surgery. TRUS, seminal MRI or CT scan for seminal vesicles were carried out to exclude tumor of seminal vesicle, tubercle and carcinoma of prostate. Seminal vesicle endoscopy was conducted using urethra F4.5/6 ureteroscope, old coagulated blood in seminal vesicles was washed. Holmium laser was used to break calculus of seminal vesicle and quinolones was perfused into the seminal vesicles during the surgery. Four weeks finasteride therapy followed after operation. Results For 19 patients, 18 patients successfully received bilateral seminal vesicles endoscopy, washing and drug reserved perfusion. One patient completed injured seminal vesicle endoscopy and treatment. Six months to one year follow-up indicated that no hemospermia was found in 18 patients and one patient was diagnosed with hemospermia recurrence. The symptoms were improved after sensitive antibiotics anti-inflammatory treatment. Conclusion Seminal vesicle endoscopy per urethra using F4.5/6 ureteroscopes is a safe and effective method to treat refractory hemospermia.

Abstract:
Objective To prepare high purity pharmaceutically acceptable sodium aescinate from Chinese buckeye seed and investigate its therapeutic effect on tissue edema rats. Methods Sodium aescinate was firstly roughly extracted from Chinese buckeye seeds by ethanol and decolorized with activated carbon. Then, the extraction products were further purified by ion-exchange chromatography by loading onto a column packed with ion-exchanger medium. The final sodium aescinate products were used as spraying agent to treat tissue edema rat model. Results The purity of sodium aescinate was over 90% after extracted by ethanol and purified via ion-exchange chromatography. When used as spraying agent, sodium aescinate significantly attenuated the degree of edema in the hind limbs of rats, the lesions became smaller, the infiltration of inflammatory cells and fibroblasts was apparently reduced, and the effect was equal or even better than that of the positive control Yunnan Baiyao. Conclusion Soft sepharose beads which are traditionally used for protein purification can be effectively used for Traditional Chinese medicine monomer preparation. Sodium aescinate may be a potential agent for tissue edema treatment.

Abstract:
Objective To establish a method for rapid detection of tumor serum marker CA125 antigen based on aptamer. Methods CA125 antigen was used as the screening target, CA199 antigen was used as the anti-screening target, and carboxylated agar magnetic beads were used as the screening medium. The phylogenetic technique of exponential enrichment ligand was used to screen the high specificity aptamer, and the bioinformatics method was used. The sequence analysis and secondary structure prediction of the aptamer were carried out, the affinity of the aptamer was analyzed by fluorescence quantification, and the specificity of the aptamer was determined by the ELONA method. Results After 6 rounds of SELEX screening, the affinity of the secondary ssDNA library was gradually increased, and the specific secondary ssDNA library was enriched round-by-round. After sequencing by the second generation, three nucleic acid sequences with copy number >1000 were obtained (Apt-1, Apt-2, Apt-3), can specifically recognize CA125 antigen, and determine Kd values, which are 41.02±4.9 nmol·L –1, 19.06±9.0 nmol·L–1, 10.02±5.0 nmol·L–1, Apt-3 the highest affinity. Conclusion The functional aptamer Apt-3 can highly recognize CA125 antigen, which provides a new, rapid and effective tool and means for the detection of tumor serum marker CA125.
Screening and identification of key lncRNA associate with carcinogenesis and development of CRC: a bioinformatic analysis of TCGA

Abstract:
Objective To expore the lncRNA expression profiles and features in CRC tissues and their potential value associated with prognosis assessment. Methods The differential expression, functional and pathway enrichment, interaction network and lncRNA-mRNA co-expression of lncRNA wereanalyzed using RNA-seq data from TCGA. The prognosis changes of CRC patients induced by the differential expression level of lncRNA wereanalyzed . Results TCGA RNA-seq data from 453 specimens of CRC patients showed that there were 119 lncRNA which expressed differentially. lncRNA and coding gene had the relation of co-expression. lncRNA played various roles in biological features of cells. There existed lncRNA-miRNA-mRNA regulatory network. 16 of these lncRNA had potential value on the prognosis of CRC patients, 3 of them were key lncRNA, which were BVES-AS1, HAND2-AS1 and MAMDC2-AS1. They could be seemed as the biomarkers for prognosis of CRC patients. They also had close relations with the miRNA and coding gene. Conclusion lncRNA express differently in CRC tissues, and play an important role in biological feature of CRC cells, which have potential value in diagnosis, targeted therapy and prognosis of CRC patients.
Protective effect and cellular immune response induced by recombinant Bb-OprH vaccine of Pseudomonas aeruginosa

Abstract:
Objective To investigate protection and changes of splenocyte proliferation, apoptosis and subsets after immunization with recombinant Bifidobacterium (rBb)-OprH vaccine of Pseudomonas aeruginosa (Pa). Methods Balb/c mice were vaccinated intragastrically with the vaccine and challenged by Pa PAO1 strain intranasally at the 4th week after the first immunization. The mice were sacrificed two weeks after the challenge to isolate their lungs and spleens, and the pulmonary bacterial loads were counted. MTT assay was used to detect splenocyte proliferation, while apoptic rate and subsets of splenocytes were measured by flow cytometry. Results After immunization with the vaccine and challenged by Pa PAO1 strain, the bacterial loads in the lungs decreased, while the splenocyte proliferation and the number of CD4+ T cells increased. The apoptic rate decreased. Conclusion The recombinant Bb-OprH vaccine can induce a protective cellular immune response in mice against the Pseudomonas aeruginosa.
A preliminary study of conditional limb bud mesenchymal stem cell FKBP38 knockout mice

Abstract:
Objective To construct and identify conditional limb bud mesenchymal stem cell FKBP38 knockout mice, and provide an animal model for the mechanism of FKBP38 gene in bone development and osteoarthritis. Methods Prrx1-Cre mice were mated with FKBP38flox/flox to produce F1 mice. The genotype was Prrx1-Cre(Fkbp38flox/+) mice. The mice with genotype Prrx1-Cre(Fkbp38flox/+) were selected. FKBP38flox/flox mice were crossed to obtain F2 mice. Mice with genotype Prrx1-Cre(Fkbp38flox/flox) were used to construct model mice for the experiment. The mouse genotype was identified at 3 weeks of age. The knockdown effect of FKBP38 gene was verified by Western blot at 4 weeks of age. Results F2 mice contained knockout mice. The PCR results showed that the genotype was Cre/FKBP38flox/flox. The results of Western blot showed that the FKBP38 gene knockout effect was obvious. Conclusion Based on the Cre/loxp system, the limb bud mesenchymal stem cell FKBP38 knockout mouse is propagated to provide a model basis for exploring the mechanism of FKBP38 in bone development and osteoarthritis at the animal level.

Abstract:
Papillary thyroid carcinoma originated in thyroid follicular epithelial cells, the prognosis is generally better. However, the incidence of PTC has risen sharply in recent years, seriously affecting public health. Papillary thyroid carcinoma is often divided into several subtypes: Classical, Follicular-variant, Tall cell subtype, Hobnail variant and Diffuse sclerosis, et al. The aggressiveness of different subtypes is different. BRAFV600E mutation is the most common molecular marker of papillary thyroid carcinoma and is also a hot spot in molecular research in recent years. It is closely related to the occurrence, development mechanism, invasion and prognosis of PTC. With the deepening of BRAF V600E mutation research and its contradictory points and controversies about the relationship between PTC and BRAF V600E, this review summarizes the relevant literature in the recent five years mainly from the mechanisms of BRAF V600E mutation and PTC, the clinicopathological features relationship, prognosis, guiding clinical treatment and related gene combination and so on. The dissertation analyzes the controversial and contradictory points in current literatures.

Abstract:
The incidence of colorectal cancer is very high. Early diagnosis and early treatment can improve the prognosis of patients, and can increase the survival rate of patients. Therefore, early diagnosis of colorectal cancer is significant for colorectal cancer. As more people attach importance to it, various detection methods appear, and the sensitivity and specificity of various molecular markers are continuously increasing. Abstract:
In recent years, 3D bio-printing has attracted more and more attention from researchers. This technology makes up for the limitations of traditional tissue engineering technology. It can print out accurate and personalized scaffold structure, and can design shape, size, pore structure and porosity at etc. The 3D bio-printing scaffold usually provides structural support and suitable microenvironment to support cell behavior in tissue engineering. The microporous structure and porosity of the scaffold are essential to the behavior and health of the cells. Therefore, the 3D bio-printing scaffolds should be highly porous and interconnected pore networks to facilitate nutrient transportation, waste excretion, and cell growth、 proliferation and differentiation. This paper reviewed the materials and printing methods of 3D bio-printing scaffolds from inanimate to life, and focued on the study of the microporous structure and porosity of the scaffolds.

Abstract:
MicroRNAs (miRNAs) are endogenous, non-coding small RNAs of approximately 19-25 nucleotides in length, which have a variety of regulatory roles in cells.Such as regulating the proliferation, apoptosis and differentiation of different cells.etc, mainly by targeting completely or incompletely the 3′-UTR region of mRNAs affects the translation and post-transcriptional regulation of targeted gene expression. Recent studies have found that microRNA-21 was involved in the pathogenesis of non-alcoholic fatty liver disease, which is related to its progression and prognosis. Comprehensive research found that the expression of microRNA-21 can be involved in the development of non-alcoholic fatty liver disease and its related complications through multiple targets, such as HMGCR, LRP6, PPAR-α. This article reviews the role and molecular mechanism of microRNA-21 in different stages of development of non-alcoholic fatty liver disease, and provides a basis for further research on the treatment of non-alcoholic fatty liver disease.

Abstract:
Tooth development is a long-term and complicated biological process which includes the occurrence of tooth germs, the formation of dental tissues and the eruption of the tooth. It includes the development of the crown and the root. After the formation of the tooth crown, the development of the tooth begins. It is regulated by the epithelial and the ectodermal mesenchymal tissues, including the development of the dentin and the cementum. In recent decades, scholars carried out a large number of experimental studies of the development mechanism of the crown. And it has been in-depth understanding. However, the regulatory mechanism of the root is poorly understood. Recently, the discovery of dental stem cells has promoted the progress of dental tissue engineering. It is also closely related to root development. However, the mechanism is still unclear. This article reviews the development mechanism of the root and the relationship between root development and dental stem cells.

Abstract:
Atherosclerosis is a characterized by the accumulation of lipids and chronic inflammation in the large and middle-sized vascular arterial wall. The cause of Atherosclerosis and occurrence mechanism is not understood clearly. Oxidative stress is considered to be integral to the development and progression of atherosclerosis. The overproduction of reactive oxyen species (ROS) will damage the artery and promote atherosclerosis. Autophagy plays a crucial role in cell homeostasis by eradicating misfolded proteins and damaged cell organelles. Emerging evidence suggests that autophagy may be very importan in the pathogenesis of atherosclerosis. This review focus on the recent advaces of oxidative stress and autophagy in atherosclerosis, in order to further clarify the mechanism and provide the theoretical basis for targeted treatments.

Abstract:
Objective To evaluate the effect of autologous stem cells treatment on Crohn's disease. Methods Published papers and abstracts from Pubmed, MEDLINE, EBSCO, Current Content Connect, Cochrane, China Biomedical Literature Database, China HowNet, Wanfang Database, Wipe Database, Google Academy, National Library Reference and Consulting Union database were screened from February 1963 to July 2008. After independent data collection, evaluation, data extraction and cross-check were done by 2 authors. RevMan 5.3 software was used for multivariate analysis, and Q-test for identifying the heterogeneity between two groups in the study. According to the results of Q-test, random effects and fixed effects models were used and then subgroup analysis was done in this meta-analysis. Results A total of 1048 published papers were found. After both animal experiments and case reports were excluded, 13 papers were finally included, e.g. 9 retrospective studies and 4 case-control studies. Meta-analysis showed that the RD value after stem cell therapy and before therapy was significantly different (RD=0.81, P<0.00001). The OR value after stem cell therapy and before therapy was significantly different (OR=11.00,P<0.00001), but there was no significant difference of the activity index in patients with Crohn's disease between treatment group and control group (P=0.13). Conclusion Autologous stem cells therapy might improve the clinical symptoms in patients with Crohn's disease in a short time, its safety and efficacy are still unclear.

Abstract:
Objective To investigate the sedative and analgesic effects of ketamine-propofol combination and ketamine-dexmedetomidine in the extraction of children with dental anxiety Methods The study was conducted at the Department of Oral and Maxillofacial Surgery, Faculty of Dentistry, Southern Medical University, Guangzhou, China, from September 2016 to September 2017, and comprised children who were due to undergo extraction of embedded supernumerary teeth in maxillary anterior. Of the 60 participants, there were 30 (50%) in each group. Further, the ketamine-propofol group received 1mg/kg of ketamine and propofol, and the ketamine-dexmedetomidine group received 1mg/kg of ketamine + 0.5 μg/kg of dexmedetomidine. Preoperative and postoperative anxiety scores, postoperative pain scores, anesthesia recovery scores, surgeon satisfaction (bad/medium/good), additional medication required, operative time, number of extractions, and adverse events were recorded At the same time, SBP/DBP, HR, RR, RSS and SPO2 values were recorded every 5 min after induction of anesthetic drugs and recorded for 2 hours. Results No statistically significant differences were found in terms of heart rate, non-invasive blood pressures, duration of operation, anesthesia recovery time, sedation score, preoperative and postoperative anxiety score, postoperative pain score at any time and the number of drug repetitions (p>0.05). Nausea-vomiting was statistically higher in the ketamine-dexmedetomidine group (p<0.05). Conclusion There was no significant difference in sedative and analgesic effects between ketamine-propofol group and ketamine-dexmedetomidine group. However, ketamine-propofol had a lower rate of nausea and vomiting than ketamine-dexmedetomidine group, and the surgeon's satisfaction was higher. Combination of ketamine and propofol may be a satisfactory choice in the extraction of maxillary impacted supernumerary teeth in children.
Value of Neurotrophin-3 in early diagnosis of acute myocardial infarction

Abstract:
Objective To explore the value of the biochemical marker Neurotrophin-3 (NT-3) in early diagnosis of acute myocardial infarction (AMI). Methods A total of 90 chest pain patients with suspected myocardial infarction were selected. 45 patients with confirmed AMI were selected as the observation group, and 45 cases of chest pain but not AMI were selected as the control group. Serum NT-3, CK-MB and cTnT levels were measured in the two groups at <3 h, 3-6 h and 6-12 h after chest pain. The sensitivity and specificity of NT-3, CK-MB and cTnT in the diagnosis of AMI were compared with <3 h, 3-6 h and 6-12 h after AMI. Results There were no significant changes in serum NT-3, CK-MB and cTnT in the control group (P>0.05). Serum NT-3 in patients with AMI was significantly increased at <3 h after AMI (P<0.01), and increased significantly at 3-6 h and 6-12 h (P<0.01). Serum CK-MB in patients with AMI did not increase significantly within 6 h after AMI (P>0.05), but increased at 6-12 h (P<0.01).Serum cTnT did not increase significantly at<3 h after AMI (P>0.05), and increased significantly at 3-6 h and 6-12 h after AMI (P<0.01). The sensitivity and specificity of serum NT-3 were higher than CK-MB and cTnT at <3 h and 3-6 h after AMI.The sensitivity and specificity of serum NT-3 at 6-12 h after AMI were similar to those of cTnT, and the sensitivity and specificity of NT-3 and cTnT were higher than those of CK-MB. Conclusion The sensitivity and specificity of serum NT-3 in early diagnosis of AMI may be superior to traditional myocardial markers. It can be used as a new early diagnostic indicator of AMI.

Abstract:
Objective To detect the clinical effects of Gamma Globulin combined with nasal continuous positive airway pressure(NCPAP) in treating acute neonatal pneumonia complicated with respiratory failure. Methods We selected 100 cases of patients with severe neonatal pneumonia complicated with respiratory failure who were treated in our hospital from March 2015 to March 2018.According to the random number table method, they were randomly divided into the control group and the observation group, 50 cases in each group. Patients in 2 groups were all given comprehensive treatment including anti-infection. Additionally, the control group was given NCPAP, on basis of which the observation group selected Gamma Globulin to treat. The clinical manifestations before and after treatment, two groups blood gas analysis, C-reactive protein, tumor necrosis factor-α, interleukin-1β immunoglobulin levels before and after treatment and complications of two groups were compared. Results After the observation group was treated by NCPAP and Gamma Globulin for 24 h, the blood gas analysis index of the observation group after 24 h was significantly improved than that of control group (P <0.05) . The relieving time of dyspnea, heart rate recovery to normal and the time of the disappearing of rales of lungs were significantly lower than that of control group ( P<0.05). The expressions of C-reactive protein, tumor necrosis factor-α, interleukin -1β were lower than control group (P<0.05). Serum IgG, IgA and IgM levels were significantly increased than that of the control group (P<0.05). Conclusion It has distinct clinical effect to apply NCPAP with Gamma Globulin in the treatment of acute neonatal pneumonia complicated with respiratory failure. It can effectively improve neonatal condition, shorten the symptoms and signs of disappearance time, reduce the level of inflammatory cytokines, improve immunity.

Abstract:
Objective To explore the effect of valproate and oxcarbazepine on oxidative stress in benign childhood epilepsy with centrotemporal spikes (BECT) children. Methods Thirty healthy children were assigned to the control group, and 64 newly diagnosed BECT children were assigned to the experimental group. Moreover, 64 newly diagnosed BECT children in the experimental group were randomly assigned to the valproate (VPA) group (n=32) and the oxcarbazepine (OXC) group (n=32). Children in the VPA group and OXC group received VPA and OXC treatment, respectively. Nitric oxide (NO), malondialdehyde (MDA) and xanthine oxidase (XO) were detected before and 3 months and 6 months after treatment. Results NO, MDA and XO levels in the experimental group were significantly higher than that in the control group (P<0.05). There were no significant differences in NO, MDA and XO levels among before and 3 months and 6 months after VPA treatment (P>0.05). In the OXC group, NO, MDA and XO levels were significantly higher after 3 months and 6 months than that before treatment (P<0.05). Conclusion Oxidative stress is higher in BECT children. OXC could significantly reduce oxidative stress in BECT children while VPA could not, which is worth popularizing clinically.

Abstract:
Objective To explore the clinical value of bundles of care in cerebral resuscitation after cardio-pulmonary resuscitation and some related biochemical index of cerebrospinal fluid difference. Method A total of 160 cardiac arrest cases from 2015 to 2018 were divided into two groups based on different management method after cardio-pulmonary resuscitation: control group (normal management) and experimental group (bundles of care). The satisfaction degree of cerebral resuscitation between two groups was compared. The clinical value of bundles of care in clinical practicing was evaluated. The NSE、S-100β、ICAM-1、FIB level in cerebrospinal fluid in two groups were checked after treatment. Results The satisfaction degree of experimental group was significantly higher than that of control group (Well satisfied: χ2=6.328, P=0.012; More satisfied: χ2=3.999, P=0.046). The level of NSE、S-100β、ICAM-1、FIB of cerebrospinal fluid were lower than that of control group. Conclusion Bundles of care is worthy of using in clinical cardio-pulmonary resuscitation. It could increase the satisfaction degree of cerebral resuscitation after cardio-pulmonary resuscitation. Bundles of care can improve the circumstance of related biochemical index in cerebrospinal fluid.

Abstract:
Objective To screen the in indicator for expectation of health care. Method Three rounds Delphi consultations were conducted for selected indicators of expectation of health care.25 experts and 25 residents were selected for the three rounds’consultations. Results The participation coefficient of Pre-survey, first round and second round consultation are greater than 70%. The average coefficient of experts’authority were 0.803. The consistency coefficients of experts were all more than 0.7 in the last round. The advice of experts were more and more consistency in the three rounds. The level of consistency reached the level, which research requested. Finally, 52 items were contained for further analysis. Conclusion The result of the consultation as the previous expected and the indicators have a high reprehensive and scientificity.

Objective To understand the knowledge and evaluation of mixed-ownership medical institutions by domestic experts and scholars, and to provide decision-making reference for promoting the mode of mixed ownership. Methods A questionnaire survey was conducted to conduct in-depth interviews with 31 experts. Results Most scholars are in favor of exploring mixed-ownership cooperative medical treatment, which is considered to be conducive to the formation of a multi-disciplinary medical model; the lack of government financial input to promote the use of social capital to expand the development of the attribution analysis of public hospitals is the consensus of most experts; Multi-party goals, we must focus on the interests of cooperative stakeholders to achieve win-win cooperation as much as possible; more than half of the experts believe that mixed-ownership medical institutions are determined to be non-profit more appropriate; most experts believe that income and expenditure balances are not dividends; most experts believe that There is not much inevitable relationship between the introduction of social capital and the changes or adjustments of hospital development strategies; experts generally believe that government governments should give full play to the regulatory role of the industry and build a good external policy environment for mixed ownership. Conclusion The mixed ownership model should be affirmed and encouraged, because it can introduce a competitive mechanism to form a multi-disciplinary medical environment, alleviate the insufficiency of government funding, meet the growing health needs of the people, and promote the establishment of a modern hospital management system.

Abstract:
Objective To provide the suggestion of popularizing the family doctor policy by investigating the applicability of the family doctor service in inrural areas. Methods From July 2017 to August 2017, a questionnaire survey was conducted among rural residents in Nanbu County of Nanchong, Sichuan Province. We used a self-made general data questionnaire and family doctor's willingness to sign a contract questionnaire. Results Educational level had a effect on the signing service of rural residents' family doctors (Ρ<0.01). Hospital type selected by rural residents and the attractive source of family doctors' signing system were influencing factors of willingness to sign contracts with family doctor (Ρ<0.05). Conclusion Signing service with family doctors has a lot of benefits to the residents in rural areas. But it is difficult to popularize. We should make the rural residents understand the policy of contracting service of family doctors, and promote the popularization of the contracted services of family doctors in rural areas.

Abstract:
Objective To explore the effect of continuous nursing mode on the maintenance of cognitive level, self-management ability and complications of PICC in patients with leukemia discharged through peripheral venous catheterization of central venous catheter (PICC). Methods A total of 110 cases of leukemia patients with PICC at our department were selected from January 1st 2016 to May 1st 2018. These patients were divided into observation group (n=55) and control group (n=55) according to random number table. The patients with PICC in control group were applied with traditional outpatient nursing mode, while observation group were applied with continuous nursing mode. The maintain cognitive level, self-management ability and the incidence of complications between two groupswere compared. Result The cognitive level and self-management ability of PICC maintenance were significantly higher in observation group than control group (P<0.05). The incidence of complications, phlebitis, infection and conduit jam of observation group was significantly lower than control group (P<0.05). Conclusion Continuous nursing mode can increase the maintenance of cognitive level, self-management ability of PICC in patients with leukemia discharged. It reduces the incidence rate of complications of PICC.
