Correlation between Diabetes and cognitive dysfunction in the aged population

Abstract:
Objective To explore the correlation between diabetes and cognitive dysfunction and to elucidate the effect of diabetes on cognitive dysfunction in the aged population. Methods The study was a cross-sectional study. People with subjective complaints (80 years old) were approached through inpatient and outpatient neurology practices between January 2009 and December 2012. Medical History acquisition, neurological examination and neuropsychological scales assessment was conducted. The prevalence of diabetes in different cognitive function populations and the effect of diabetes on cognitive dysfunction were analyzed. Results A total of 286 aged patients were enrolled in this study, among which 60 had normal cognitive function, 85 had mild cognitive impairment and 141 had dementia. The prevalence of diabetes was 8.33% (95%CI 18.5%~30.3%) in normal cognitive group vs 23.53% (95CI14.4%~36.3%) in patients with MCI and 29.79% (95%CI 21.5%~40.3%) in patients with dementia. The prevalence of diabetes in the dementia group was significantly higher than that of in normal cognitive group (P<0.05). Diabetes decreased with the MMSE and CDT score by using Spearman Rank Order Correlation. Multivariable logistic regression analysis showed diabetes was significantly associated with essential hypertension. Conclusion The prevalence of diabetes in elderly patients with dementia was significantly higher and diabetes was significantly correlated with cognitive dysfunction. Tightly control of diabetes is very important for prevention and treatment of dementia in the aged population.
The ExPression and clinical significance of eIF4E and ki67 in patients with endometrosis

Abstract:
Objective To explore the expression and clinical significance of eIF4E and ki67 in endometriosis. Methods The expression of eIF4E and ki67 were detected by immuno- histochemical method of SP in 30 cases of endometriosis and 20 cases of normal endometrium. Results Expression of the three groups showed such sequence as follows about eIF4E and ki67: ectopic endometrium>eutopic endometrium>normal endometrium, there was a significant difference statistically (P<0.01); eIF4E during the reign of normal endometrium and endometrial secretion period is higher than the proliferation period, the differences are significant statistical significance (P<0.05), ki67 in normal secretory phase endometrium group is higher than breeding period (P<0.05), in ectopic group multiplication period is higher than the discharge (P<0.05). The eIF4E and ki67 protein had obvious positive correlation in ectopic endometrial and endometrium (r=0.861, P<0.01; r=0.788, P<0.01), but there was no correlation in the normal endometrium (r=0.297, P=0.207). Conclusion eIF4E and ki67 in endometriosis express obviously increased. The expression of eIF4E and ki67 were positively associated with development and progression of endometriosis.

Abstract:
Objective the purpose of this study is to evaluate clinical value by analyzing 3D-CDFI in bladder tumor diagnose. Methods sampling 52 focuses from 43 patients who had bladder masses from our hospital, from January 2012 to February 2014, comparing diagnosis of two-dimensional (2D) ultrasonographies, 3D-CDFI, three-dimensional (3D) ultrasonographies and cystoscope to result of 3D-CDFI clinical value. Results According to pathological and cystectomy diagnosis, 48 out of 52 bladder masses were bladder tumors. Amongst 48 bladder tumors, 45 cases proved malignant (45/48) while 3 are benign; 4 out of 52 cases are non-48 bladder tumor (3 proved to be clot and 1 proved to endometriosis). Diagnose accordance rate to endometriosis: the accordance rate of result to three-dimensional ultrasound 96.15% (50/52) is higher than two-dimensional ultrasound 85.69% (43/52), P<0.05, which is of statistical significance. The result to Color Doppler ultrasonography 3D-CDFI 85.25% (39/48) within Bladder tumor is higher than CDFI 62.50% (30/48), P<0.05, which is of statistical significance. The accordance rate of result to 3D-CDFI Staging is higher than two-dimensional ultrasound, P<0.05, which is of statistical significance. Conclusion 3D-CDFI can increase the blood flow display rates of bladder tumor, and judge the depth of bladder tumor, so it is important to precise tumor diagnosis. It is feasible for clinical application for 3D-CDFI is easy to control, non-invasive, replicable, time and money saving, and acceptable to patients.

Abstract:
Objective To investigate the injury of small intestinal mucosa induced by Non-steroidal Anti-inflammatory Drugs (NSAID). Method 76 cases of capsule endoscopy were divided into the observation group and the control group. The observation group included 40 patients with long-term use of aspirin (28 cases, 100 mg/d) and other NSAID (12 cases) for various causes. The patients in the control group did not take any NSAID. To observe and analyse intestinal mucosal bleeding, erosion and ulcer, etc. Results 23 cases of small intestinal mucosal damage occurred in the observation group (57.50%), and 7 cases also occurred in the control group (11.11%), the incidence of small intestinal mucosal damage in two groups was statistically significant different (P<0.01), However, it showed no significant difference in the aspirin group and in the other group with other NSAID (P>0.01). Conclusion long-term administration of NSAID can cause damage to the small intestinal mucosa, different types of NSAID similarly damage to the intestinal mucosa.

Abstract:
There are two case of patients with obstructive jaundice caused by NHL in our department recently. They are misdiagnosed as cholangiocarcinoma or pancreatic cancer by computed tomography. Because of their long jaundice, severe liver dysfunction, abdominal multiple lymphnode metastasis, they don't have operation like cholangiocarcinoma or pancreatic cancer. They are performed percutaneous transhepatic cholangiography and drainage catheter, then biopsy. Pathology results confirmed non-hodgkin's lymphoma. After systemic chemotherapy, the tumor are complete remission, also relieve biliary compression, jaundice subsided. Obstructive jaundice is a rare presenting feature of non-hodgkin's lymphoma, which is different from the cholangiocarcinoma or pancreatic cancer in treatment. Recently more focus oil the biopsy of the tumor at the hepatic hilum or in the pancreatic head region was helpful for the diagnosis of lymphoma.

Abstract:
Objective Using E-TRACKING (E-TRACKING) evaluation of endothelial cell damage in type 2 diabetic patients, to explore the relationship between glycated hemoglobin. Methods 30 healthy people, blood glucose control in type 2 diabetes normal in 14 cases, 16 cases of abnormal glucose control, evaluation of patients with arteriosclerosis index, pressure-strain elastic parameters coefficient (Eр), stiffness (β), compliance (AC), pulse wave velocity (PWVβ) differences swelling index (AI) between comparing the relationship between glycated hemoglobin and determine atherosclerosis in diabetic patients and HA1c relationship between the degree of hardening. Results ① Compared with normal control group, the common carotid artery β diabetic group, Eр, PWVβ were increased, the difference was statistically significant; while AC decreased significantly, AI increased, no significant difference in differences. ② Compared with the normal group, glycated hemoglobin, glycated hemoglobin carotid artery increased β group, Eр, PWVβ were increased, the difference was statistically significant; while AC decreased significantly, AI increased, but there was no significant difference in differences. Conclusion The patients with type 2 diabetes can be evaluated eTRACKING early atherosclerosis extent, contribute to early treatment of atherosclerosis progress slow.

Abstract:
Objective To explore the feasibility and conclude experiment of an operation in which removing the injection augmentation agent polyacrylamide hydrogel under minimally invasive surgery. Methods All 58 breasts in 35 patients were removed PAHG, 52 breasts in 32 patients with endoscope-assisted and 5 breasts in 3 patients whose injection located in the subcutaneous or mammary gland were removed by mammotome. It has draw a conclusion about the selection of the best incision and effect and experiment of operation. Results A majority of polyacrylamide hydrogel be removed under endoscope-assisted in 52 breast. Follow-up 3 to 6 months by high frequency ultrasound, showed no discovered polyacrylamide hydrogel in 48 breasts, but in 4 breasts were discovered located in the subcutaneous and 2 of them complicated with no echo nodules in mammary gland. 5 breasts revomed the injection by mammotome, follow-up 3 to 6 months showed no discovered. Conclusion A majority of polyacrylamide hydrogel may be removed under endoscope-assisted. In cases of few polyacrylamide hydrogel left in mammary gland or subcutaneous, can be removed by mammotome. It is safe and feasible and gives good cosmetic results remove of polyacrylamide hydrogel under minimally invasive surgery. We believe that offers other selections of removing breast polyacrylamide hydrogel injection.

Abstract:
Objective analyze the clinical distribution, constitution and antimicrobial resistance of staphylococcus aureus as to selectantimicrobial agents correctly and efficiently. Method Totally 290 strains of staphylococcus aureus is isolated from various specimens were collected, and the species identification and drug susceptibility test were performed by the VITEK 2 compact automatic bacteria identification instrument. Results There were 290 strains of staphylococcus aureus isolated in 2 years, the main types include, Sputum 106 strains (36.5%), Wounds 95 strains (32.8%), Blood 53 strains (18.3%). Mainly distributed in the Department of acupuncture and moxibustion in 41 strains, vasculitis section 34 strains, nephrology 28 strains, endocrinology 28 strains, encephalopathy section 24 strains, ICU 22 strains, of 17 strains of lung disease, penicillin G, erythromycin, tetracycline, ciprofloxacin, clindamycin, gentamicin resistance rates were 92.8%, 65%, 62.1%, 57.5%, 48.7%, 47.9%. Cotrimoxazole, Rifampicin, Nitrofurantoin resistance rates of less than 20%, Vancomycin, Linezolid, Tigecycline, Sensitive to teicoplanin was 100.0%. 290 staphylococcus aureus detection methicillin-resistant staphylococcus aureus (MRSA) 174 strains (60.0%). Conclusion It is important significance to understand and master the distribution and epidemiology of this hospital Staphylococcus aureus and resistance changes, to guide the clinical rational use of antimicrobial drugs, delay the acteria resistant strains.

Abstract:
Objective To investigate the occurrence and treatment of acute organophosphorus pesticide poisoning with respiratory failure, improve the level of clinical emergency. Methods 42 cases of acute organophosphorus pesticide poisoning were analyzed and summarized the clinical. Results Cured 36 cases, 4 deaths, mortality 9.52%, which use continuous renal replacement therapy (CRRT) in 6 patients without a death. Conclusions Increased rate of respiratory failure, the key to successful treatment is to prevent respiratory muscle paralysis, the emphasis on mechanical ventilation and CRRT combined therapy.

Abstract:
Objective To explore the treatment of ankle joint fracture absorbable screw with inferior tibiofibular syndesmosis separation in. Methods 2009 August ~2014 August for the treatment of ankle fractures in 150 cases, were treated by open reduction and internal fixation, which accompanied with separation of distal tibiofibular joint in 48 cases, fixation, Carm fluoroscopy, such as the discovery of tibiofibular syndesmosis separation line still, absorbable screws fixed transverse. Result all cases were followed from 6 months to 4 years and 9 months follow-up, an average of 2 years and 4 months. According to the American Association of foot and ankle surgery(AOFAS)evaluation of ankle joint standard, excellent in 37 cases, good in 9 cases, in 1 cases, poor in 1 cases. The excellent and good rate was 95.8%. Conclusion the absorbable screws in treatment of fractures of the ankle, with fixed characteristics and microstructure of good internal consistency, early functional exercise, reducing reoperation demolition of horizontal screw surgery pain, to avoid the risk of metal screw fracture.

Abstract:
Objective Tostudy the reference data of CTV referential safety margins of PTV in Three-dimensional Conformal Radiotherapy target volume. Methods The translational deviation of 247 patients during Jan~Dec. in 2013 were examined in this study. Thermoplastic mask was used for head and neck and vacuum cushion was used for chest and abdomen immobilization. The verification films were taken for each patient prior the first treatment and the Nth were compared with the digitally reconstructed radiographs (DRR) by the HW-TPSQA system, in order to make translational deviation. CTV margin should be calculated with the formula 2.5Σ+0.7σ by Van Herk al. Σ means Systematic Error, σ means Random Error. Result The margin of PTV of head and neck in three directions (X, Y, Z) was 1.09 mm, 0.62 mm, 2.42 mm respectively, the margin of PTV of chest in three directions (X,Y, Z) was 2.36 mm,4.04 mm,3.69 mm and the margin of PTV of abdomen in three directions (X, Y, Z) was 2.79 mm, 2.23 mm, 3.11 mm. Conclusion It has laid a theoretic foundation for our CTV margins by analyzing the translational deviation and made our design of radiotherapy system more scientific and normal in Three-dimensional Conformal Radiotherapy target volume.

Abstract:
Objective To explore the risk factors for persistent hypertension after the puerperium in pregnancies complicated with hypertensive disorders. Methods 60 cases who suffered hypertensive disorders in pregnancies were included as cases group. Controls, matched 2∶1 to cases, were selected from patients who did not develop hypertensive disorders. Related indexes were compared between the two groups and multiple Logistic regression analysis was applied to the confirmed indexes. Results 154 patients completed the postpartum 6-week follow-up, 103 cases in cases group and 51 cases in control group. The average age was 28.5±1.3 and the morbidity was 33.1%. There were significant differences between the two groups in age, uric acid and creatinine (P<0.05). Furthermore, age≥ 35, elevated uric acid and elevated creatinine were the independent risk factors of postpartum persistent hypertension based on multiple Logistic regression analysis (P<0.05). Conclusion Intensive surveillance and control should applied to postpartum blood pressure in pregnancy-induced hypertension with age≥ 35, elevated uric acid and elevated creatinine.

Abstract:
Objective To investigate the outcome efficiency of internal fixation using mini titanium plate for treatment of radius head fracture with Mason Ⅱ or Ⅲ type. Methods Radius head fracture with Mason Ⅱ or Ⅲ type in 30 patients were treated by open reduction and internal fixation with mini titanium plate. In these 30 cases, 3 patients adopted allograft bone treatment, and collateral ligaments were repaired in 2 cases. Result Bone healing occurred in all 25 cases which were followed for average 15 months from 10 to 29 months. No deformations were observed in these cases after operation. The functional evaluation scale of elbow was average 93 (from 70 to 100), the angle of elbow supination was average 60° (from 40° to 85°). In this group, the excellent and good rate of outcome is 92% with excellent in 18 cases, and the good in 5 cases respectively. There was one patient's muscle force was weaker but no effect to elbow function, and mild elbow pain occurred in another patient. Conclusion The outcome of internal fixation using mini titanium plate for treatment of radius head fracture with Mason Ⅱ or Ⅲ type is satisfied with security of internal fixation, early exercise of joint and decreasing of disability rate.

Abstract:
Nitrotyrosine is a product of of tyrosine nitration mediated by peroxynitrite anion and nitrogen dioxide, which is well acknowledged as a significant factor in diabetes mellitus(DM). It is regarded as a marker of oxidative stress(OS) and a risk predictor of chronic complications of DM to assist clinical diagnosis and therapy. Present researches have found that NT has its own pathological activities, such as causing damages of protein, mitochondria and cell. Thus, in this paper, generation pathology effect of NT, and its role in establishment and development in DM are reviewed.

Abstract:
Prostate cancer is currently one of the most common urinary system tumors, the incidence of prostate cancer in China keeps rising. Treatment of prostate cancer is drawing attention increasingly. High intensity focused ultrasound (HIFU) is a novel technique, which can destroy target tissue when ultrasound beam focus on it. Reportedly, HIFU has been applied to effectively treat localized prostate cancer alone. Meanwhile, HIFU also showed certain therapy effect for some special prostate cancers, such as hormone independent prostate cancer, recurrence after radiotherapy or HIFU.

Abstract:
ObjectiveTo tudy the effect of health education on tumor patients during daytime chemotherapy.Method94 patients from Radiotherapy Department of Nanfang hospital were divided into symptom group (>50 points) and asymptomatic group(≤50 points) according to the anxiety and depression score of patients after daytime chemotherapy. The nursing way, physical condition, including gastrointestinal reaction and the droop rate of white blood cells, and satisfaction of nursing work were investigated retrospectively.ResultsHealth education in the asymptomatic group was greater than the symptoms group (P<0.05). The incidence of gastrointestinal reaction had no difference between two groups, while the droop rate of white blood cells in asymptomatic group was higher than that of symptom group (P<0.05). In addition, the satisfaction with nursing work of asymptomatic group was markedly higher than that of symptom group.ConclusionHealth education significantly relieves the anxiety and depression during daytime chemotherapy, while it also improves the satisfaction with nursing service and promotes health of tumor patients.

ObjectiveSummary perioperative nursing experience of radical gastrectomy.Methodsretrospective analysis the perioperative clinical care data of 209 patients with radical gastrectomy fromJanuary 2008 to October 2013 in General Surgery of Liuzhou Railway center Hospital and the gastrointestinal surgical of Second Affiliated Hospital of Kunming Medical University.Results209 cases of gastric cancer surgery patients, including 6 cases of postoperative gastrointestinal bleeding, 2 cases of anastomotic bleeding in, 2 cases of anastomotic leakage, 3 cases of gastroparesis, 2 cases of pancreatitis, after careful care and treatment, death 0 cases, cured 209 cases.ConclusionBy strengthening perioperative care, effective prevention of complications, and dietary guidance to help patients gradually return to normal diet, reducing patients pain and improving the quality of life of patients.

Abstract:
Hospice is a category of medical ethics, and its development means a lot to social harmony. Hospice care is introduced to China last 80 s' and still be its initial development both in theory and practice. This paper starts from a poll on Hospice care understanding of medical students, finds there are misjudge and misunderstand, and analyzes the problems during its teaching, and tries to find its solution to make sure the future medical worker contacting the most human life have a comprehensive understanding towards Hospice and promote its development in China.

Abstract:
Objective Toevaluate the efficacy of plan-do-check-act (PDCA) cycle management in standardizing antimicrobial application in aseptic operation. Method Patients with aseptic operation between June 2011 to April 2012 were selected as pre-PDCA group, while the patients between May to December 2012 were selscted as control group. According to plan, do, check and act procedure of PDCA cycle, antimicrobial use in aseptic operation were managed and controlled, case control study on the rationality of antimicrobial use before and after the implementation of PDCA were performed. Results After implementing three cycles of PDCA, antimicrobial drug use rate, drug selection, postoperative drug use, usage, drug combination and soon all showed significant improvement, and has a tendency to keep. Conclusion The PDCA cycle is an effective means for standardization the rational use of antimicrobial agents in aseptic operation.
