Application of 18F-FDG PET/CT in diagnosis of bone metastases of unknown primary tumor

Abstract:
Objective To investigate the clinical value of 18F-fluorodeoxyglucose(18F-FDG) positron emission tomography/computed tomography(PET/CT) in diagnosis of bone metastases of unknown primary tumor(BMUPT), and summarize the features of PET/CT findings in patients with BMUPT. Methods The imaging data of 49 patients with BMUPT in our hospital were retrospectively analyzed. Primary tumor sites, lymph nodes metastases, organ metastases and bone metastases were recorded. The maximum standardized uptake value(SUVmax) of each bone metastasis were determined and the SUVmax of different bone metastases properties and metastases sites were statistically analyzed. Results Forty-eight cases of primary tumor were correctly detected by PET/CT, 1 case was not correctly detected. 42 cases were newly found with lymph nodes metastases and 26 cases were found with organ metastasis. Among the 49 patients, there were 9 cases of osteogenic metastases, 33 cases of osteolytic metastases and 7 cases of mixed metastases.Incidence of bone metastasis in each place was spine>pelvis>chest>limbs>skull. The SUVmax of osteolytic metastases was higher than that of osteogenic metastases(t=3.55, P<0.05). Differences of SUVmax between osteogenic and osteolytic metastasis at the sites of bone metastases was not significant(P>0.05). Conclusion PET/CT has a high accuracy in diagnosis for a primary tumor of BMUPT.It can find new lymph nodes and organ metastases, which provides more comprehensive value for the evaluation and treatment of BMUPT patients.

Abstract:
Objective To evaluate the imaging diagnosis value and the clinical significance of 64-row 128-slice spectrum CT angiography in aortic dissection. Methods The clinical data of 123 cases with aortic dissection covering a period from 2013 to 2017 were analyzed retrospectively. Enhancement scanning was performed in each patient. All scanning images were post-processing with multi-planar reformation, volume rendering, maximum intensity projection, surface-shaded display. so that the CT signs such as the intimal flap, the true and false lumen, the intimal tear, the re-entry tears, the involved branch of aortic and the thrombus of aortic dissection could be displayed in detail. Results According to DeBakey type, there were 43 cases (35%) of type I, 6 cases (5%) of type II, 74 cases (60%) of type III. Conclusion MSCT and CT Angiography(CTA) has advantages of wide coverage, great display of small branches, and being accurately and rapidly in diagnosing and evaluating aortic dissection, It provids important imaging information for the treatment of patients.
Value of various CT findings in the differential diagnosis of peripheral lung cancer and tuberculosis

Abstract:
Objective To explore the value of various CT findings of peripheral lung cancer and tuberculosis. Methods Diverse CT findings of 58 cases of lung cancer and 32 cases of pulmonary tuberculosis which was confirmed by pathology were retrospective analyzed. Patient with lung cancer was case group, and patient with tuberculosis was control group. Valuable signs were extracted and compared between 2 groups. Results Lung cancer group had 46 cases of adenocarcinoma, 8 cases of squamous cell carcinoma and 4 cases of small cell carcinoma.The main signs were: 49 cases(84.5%)with lobulate, 39(67.2%) with burr, 20 cases(34.4%) with pleural indentation, 13(22.4%) with halo sign .In main signs of tuberculosis,pleural reaction was 26 (81.25%), the satellite focal point 22 (68.75%), calcification 22 (68.75%), burr 26 (81.25%).Difference of lobulate, halo, pleural depression, calcification, satellite focal point and pleural reaction between lung cancer group and tuberculosis group were significant (P<0.05). The diagnostic index of lobule of lung cancer was >150%, the diagnostic indexes of tuberculosis calcification、satellite focal area and pleural membrane reaction were all > 150%, which had a diagnostic value. Conclusion In the differential diagnosis of patients with peripheral lung cancer and tuberculosis, the CT signs such as lobulate, pleural depression, halo, calcification, satellite focal point and pleural reaction have a certain clinical value for the diagnosis of ung cancer and tuberculosis.
Dosimetric analysis for the number of IMRT beams to pelvic bone for postoperative pelvic tumors

Abstract:
Objective To investigate the dosimetric advantages of IMRT plans which regard pelvic bone as an organ at risk, and explore the dosimetric effect of the beam number on pelvic bone during cervical IMRT plans. Methods The target areas of 18 cases of cervical cancer patients were depicted after CT scans. 6Mv X-ray and multi-field coplanar plan were performed. Four coplanar plans were performed on every case, including IMRT7f、BMS-IMRT5f、BMS-IMRT7f and BMS-IMRT9f. The pelvic bones were not considered as organ at risk in IMRT7f.While, they were regarded as organ at risk in BMS-IMRT5f、BMS-IMRT7f and BMS-IMRT9f. The comparison between IMRT7f and BMS-IMRT7f was used to analyze the effect of different treatment plans in conformability and homogeneity of PTV and dose distribution in organs at risk. The comparisons of BMS-IMRT5f, BMS-IMRT7f and BMS-IMRT9f were used to investigate the dosimetric difference of the beam number on cervical IMRT plans. Results Compared with IMRT7f, BMS-IMRT7f had more compact isodose lines in the pelvic bone region. The difference between BMS-IMRT7f and IMRT7f in conformability and homogeneity of PTV was not significant. BMS-IMRT7f was better than IMRT7f in pelvic bone protection with significant difference. Both of BMS-IMRT9f and BMS-IMRT7f had better coverage and homogeneity of PTV than BMS-IMRT5f. The difference between BMS-IMRT5f and BMS-IMRT7f in pelvis and intestinal protection was not significant. BMS-IMRT9f had a large filed in lower dose area, while it had a small field in higher dose area. In terms of rectal protection, BMS-IMRT7f had a minimum filed size on rectum. BMS-IMRT9f was best protected against the bladder, followed by BMS-IMRT7f. The number of BMS-IMRT5f hops was the least, and the number of BMS-IMRT9f hops was the largest. Conclusion BMS-IMRT7f protects the pelvis.It increases the high dose of the rectum, small intestine and bladder without sacrificing the conformability and uniformity of PTV. BMS-IMRT7f is superior to BMS-IMRT5f in PTV conformance, homogeneity and organ protection. BMS-IMRT9f hops more with longer treatment time. BMS-IMRT7f is more suitable for clinical use in terms of dose and actual implementation efficiency.

Abstract:
Objective To explore the diagnosis and localization of focal cortical dysplasia type I, and provide the basis for the surgical treatment. Methods Video EEG with combination of DTI were performed.VEEG specific performance, ADC value, fractional anisotropy, diffusion tensor imaging were used to diagnose and locate FCD I type with negative imaging . Results The apparent diffusion coefficient of FCD region was significantly higher than contralateral area.Fractional anisotropy was significantly lower than contralateral area (P<0.05). All patients were performed with resection based on FCD position and range, which determined by VEEG and DTI . The pathological return was type of FCD Ⅰ. The patients were followed up for 12-26 months,with 7 cases of Engel's grade Ⅰa, 28 cases of grade Ⅰb, 16 cases of gradeⅡ. Conclusion VEEG combined with DTI technology can accurately define and locate FCDⅠtype with negative imaging.It can provide protection for the precise excision of epileptogenic focus and improve seizure control.
Value of MSCT in diagnosis of common parotid benign tumor

Abstract:
Objective To explore the value of MSCT in the diagnosis of common benign parotid tumors. Methods CT findings of 30 cases with parotid common benign tumor proved by pathology were analyzed retrospectively. All cases received plain and enhanced CT scanning.CT appearances between pleomorphic adenoma, gland lymphoma and BCA were compared. The image features were analyzed and evaluated. Results Sixteen cases were parotid pleomorphic adenoma, 10 cases were adenolymphoma and 4 cases were parotid basal cell tumor. A gradually increasing intensification of contrast was showed typically on pleomorphic adenomas. Adenolymphoma showed a typical intensification of contrast " quickly-in and quickly-out”, reached the peak time of enhancement at arterial phase. Basal cell tumor had a significant enhancement in the arterial and venous phase. The enhancement methods were significantly different among three tumors (P<0.05). The differences of enhancement in 3 groups were significant (P<0.05). Conclusion The parotid common benigns have characteristic CT signs. MSCT is important in the diagnosis and differential diagnosis.

Abstract:
Pleomorphic adenomas are rare in low-aged patients. It almost occur in parotid gland with high recurrence and malignant potential. Pleomorphic adenomas in children have a higher recurrence rate. Clinical diagnosis and treatment of pleomorphic adenomas is important. This report presents a case of pleomorphic adenoma of parotid gland in a 14 year old male patient. A brief review of literatures is analyzed. Early detection, a suitable surgical procedure, and regular follow-up are the key to decrease the recurrence rate for young patients.

Abstract:
Objective To exlpore the value of hippocampal volume in diagnosising of diabetic mild cognitive impairment. Methods Sixty-five cases were assigned to 4 groups: 25 normal cases in control group, 20 patients with diabetes mellitus (DM) in DM group, and 20 DM patients with mild cognitive impairment (MCI) in DM+MCI group. Cognitive function was assessed by MoCA scale. All the subjects underwent magnetic resonance imaging (MRI) to calculate hippocampal volumes. Results For left hippocampal volume, the area under the ROC curve was 2.845 cm3. For right hippocampal volume, the area under the ROC curvewas 2.875 cm3. Conclusion The volume of the hippocampus decreases in DCI patients significantly.It has a clinical value in diagnosis of diabetes mellitus with diabetic mild cognitive impairment.

Abstract:
The incidence of nasopharyngeal carcinoma in pregnancy is low with less reported in previous literature. The common clinical treatments of patients with nasopharyngeal carcinoma during pregnancy induces abortion in the second trimester of pregnancy or induction of labor third trimester of pregnancy, but neither the best of both worlds. The medical records of a case of nasopharyngeal carcinoma during pregnancy is reviewed retrospectively and the diagnosis and therapy are analyzed. The paper can make reference to the clinical treatment of the related diseases.

Abstract:
Objective To investigate the therapeutic effect of the combination of artificial hip joint replacement and internal fixation with NiTi arm-encircling bone embracing plate on treatment of complex fracture of femur. Methods A 70 years-old male patient with complex fracture of femur caused by a traffic accident were received.The patient had right transcervical femoral neck fracture and transcervical fracture of middle femur. The patient were performed with artificial hip joint replacement combined with internal fixation with NiTi arm-encircling bone embracing plate. Joint activity and internal fixation were observed after operation. Results The fracture of femur was fixated well after operation. The rotation of hip joint movement was free with no dislocation. The patient had a high satisfaction. Conclusion Combination of artificial hip joint replacement and internal fixation with NiTi arm-encircling bone embracing plate has a good therapeutic effect on complex fracture of femur. It deserves clinical promotion.

Abstract:
Objective To evaluate the clinical efficacy and safety of trimetazidine combined with atorvastatin and trimetazidine alone in the treatment of coronary atherosclerotic heart disease. Methods Randomized controlled trial of trimetazidine combined with atorvastatin in treatment of coronary atherosclerosis heart disease were retrievaled from the Cochrane Library, PubMed, Embase, Wanfang, CNKI and VIP Database.The retrieval time was limited for a Library to April 2017. The literature were screened according to inclusion and exclusion criteria. Data were extracted and methodological quality wre assessed. Meta-analysis was performed through RevMan5.3 software. Results Ten studies were included and 1166 subjects were included in the study. Compared with trimetazidine alone, trimetazidine combined with atorvastatin significantly reduced LDL-C levels (MD=–0.54, 95%CI:–0.69-–0.40, P<0.00001), total cholesterol level (MD=–1.17, 95%CI:–1.30-–1.03,P<0.00001) and triglyceride (MD=–0.51, 95%CI:–0.55-–0.47,P<0.00001), and C-reactive protein (CRP) (MD=–2.40, 95%CI:–3.25-–1.55,P<0.00001).It can reduce the attack frequency of angina pectoris (MD=–1.52, 95%CI:–1.68-–1.36,P<0.00001), and each time the duration of angina pectoris(MD=–1.67, 95%CI:–1.89-–1.46,P<0.00001). Compared with trimetazidine alone, trimetazidine combined with atorvastatin significantly increased HDL-C levels (MD=0.11, 95%CI:0.06–0.1,P<0.00001). In the adverse drug reactions, the difference between the 2 groups was not significant. Conclusion The efficacy and safety of trimetazidine combined with atorvastatin in the treatment of coronary artery atherosclerotic heart disease is good. But the results need to be confirmed by more rigorous large sample, multi-center, randomized controlled trials .
Small doses of remifentanil plus propofol-etomidate anesthesia in painless gastroscopy of adult

Abstract:
Objective To explore the feasibility of small doses of remifentanil plus propofol -etomidate anesthesia in adult painless gastroscopy, and observe the effect and adverse reactions. Methods A total of 300 adult patients of awaiting painless gastroscopy were randomly divided into 3 groups : small doses of remifentanil plus propofol (P group), small doses of remifentanil plus etomidate (E group), and small doses of remifentanil plus Propofol -Etomidate (PE group). Induction time, duration time, recovery time, time to leave recovery room and SBP, DBP,HR, SpO2 in T0 (prior to anesthesia), T1 (at the time point of undergoing gastroscopy), T2 (2 minutes after the gastroscopy), T3 (at the time point of finished gastroscopy) were observed. Injection pain, muscular fibrillation, nausea and vomit, additional drugs were recorded to evaluate the anesthesia effect. Results Recovery time and time to leave recovery room of PE group and E group were shorter time. Among PE group and E group, differences of SBP, DBP,HR, SpO2 btween the T1, T2, T3 and the T0 were not significant (P>0.05). Incidence of injection pain, muscular fibrillation, nausea and vomit and additional drugs in PE group were lower (P<0.05). Satisfaction of anesthetist and gastroscopist with EP group was higher than that of P group (P<0.05). Conclusion Small doses of remifentanil plus propofol-etomidate anesthesia in adult painless gastroscopy is safe and satisfactory with a lower incidence of adverse reactions.It has a higher satisfaction of anesthetist and gastroscopist.
Expression and mechanism of miR - 429 in patient with bladder cancer

Abstract:
Objective To explore the expression and mechanism of miR-429 in bladder cancer. Methods A total of 120 cases with bladder cancer were collected from our hospital in 2015.1-2016.12. The adjacent tissues with the edge >2 cm were selected as the control. All specimens were confirmed by pathological diagnosis without preoperative radiotherapy and chemotherapy. The relative expression of miR-429 in bladder cancer and adjacent tissues were analyzed by SYBR green real-time quantitative PCR. The relative expression of miR-429 in bladder cancer carcinoma was explored and the relationship between miR-429 and clinical features was analyzed. Results The relative expression level of miR-429 in bladder cancer tissues was significantly higher than that in normal bladder tissues (P<0.05).The expression level of miR-429 was associated with tumor size, lymph node metastasis and TNM staging of bladder cancer (P<0.05). The expression level of miR-429 in tumor tissue with diameter greater than 5 cm was higher than that in tumor group with diameter less than 5 cm. The expression level of miR-429 in bladder transition group was higher than that in bladder transition group without lymph node metastasis.The expression level of miR-429 was significantly higher than that of stage Ⅰ + Ⅱ bladder cancer (P<0.05). The expression level of miR-429 was not related to the age and sex of bladder cancer patients (P>0.05). miR-429 overexpression, bladder cancer cells (T24 cell line) proliferation increased and apoptosis reduced.miR-429 knockout, bladder cancer cells (T24 cell line) proliferation reduced.The apoptosis increased. Conclusion miR-429 is important in the development and progression of bladder cancer. Expression of miR-429 can provide references for the severity and prognosis of bladder cancer.
Survival analysis for primary hepatocellular carcinoma patients with esophageal varices rupture hemorrhage who underwent endoscopic variceal ligation in different Child-Pugh level

Abstract:
Objective To explore prognosis and survival situation of patients underwent endoscopic variceal ligation for primary liver cancer complicated with esophageal varices rupture hemorrhage. Methods Seventy-four patients were included as the observation group. Fifty-three patients who did not underwent endoscopic variceal ligation were chosed as control group. Prognosis and living time between 2 groups were compared. Results During the follow-up period, hemorrhage cases in observation group was significantly higher than that of control group (54.7% vs 17.6%, P=0.000). The volume of bleeding in control group was significantly greater than that of observation group. The arithmetic mean and median of the living time in observation group were 31 and 32 months, respectively.While the control group were 21 and 16 months. Kaplan Meier-analysis showed that the difference of living time in two groups was significant (χ2=9.499, P=0.002). Patients who underwent endoscopic variceal ligation in Child-Pugh A and B level had a long interval time to rebleeding, reduced rebleeding rate and volume, and a long living time. But for the patients in Child-Pugh C level, the effect of endoscopic variceal ligation needed more evidence. Conclusion Patients with esophageal varices rupture hemorrhage in Child-Pugh A and B level should underwent endoscopic variceal ligation as early as possible. Survival time of patients in Child-Pugh C level underwent endoscopic variceal ligation will not be shortened.

Abstract:
Chondrosarcoma is a common malignant bone tumor, and the treatment methods include amputation and limb salvage surgery.The paper reported a case of humerus chondrosarcoma treated with alcohol inactivated autograft replantation. The patient was followed up for 12 months, with no recurrence and metastasis of the tumor. The treatment and indications of malignant tumors of humerus were analyzed to provide more reference for the clinical treatment of chondrosarcoma.

Abstract:
Objective To explore risk factors of steatosis degree in patients with type 2 diabetes detected by transient elastography. Methods Clinical data of 120 patients with type 2 diabetes in our hospital from January 2015 to May 2016 were collected. According to CAP and alcohol consumption, patients were divided into 2 groups: normal group of CAP<238 db/m, nonalcoholic fatty liver group (NAFLD group) of CAP≥238 db/m group. Age, gender, weight, height, BMI, duration, systolic blood pressure, diastolic blood pressure, total cholestero, triglycerides, high density lipoprotein, low density lipoprotein, blood urea nitrogen, creatinine, uric acid, glycated hemoglobin, controlled attenuation parameter (CAP) in two groups were compared. Results Weight of NAFLD group was higher than that of normal group (P<0.05). Multiple linear regression analysis showed, CAP and weight of T2DM patients were correlated (r=0.726). Regression equation obtained: Y=77.417+2.503X (Y for the CAP, X body weight). Conclusion Weight is a independent risk factors for steatosis degree in patiens with T2DM.Steatosis and body weight has a linear correlation.

Abstract:
Objective To explore the application of two detection methods of chromosome karyotype analysis and fluorescence in situ hybridization (FISH) in the diagnosis of fetal etiology. Methods Chromosome karyotype analysis was performed in 78 cases of pregnancy after embryo damage.If the culture fails, fluorescence in situ hybridization (FISH) was used. Results The pregnancy tissue of 78 patients with embryo abortion, chromosome culture and successful use in 38 cases of karyotype analysis. Among them, there were 23 normal karyotypes, 15 abnormal karyotypes, 8 cases of autosomal trisomy, 1 case of triploid, 3 cases of sexual chromosome abnormality, and 3 structural abnormalities.Forty cases of chromosomal failure were detected by FISH, 15 of the normal chromosomes were detected. Twenty-five cases of abnormal chromosome, with 19 cases of autosomal trisomy, 2 cases of triploid and 4 cases of 45XO. Conclusion The main cause of embryo abortion is the abnormal chromosome of the embryo. The pregnancy tissues are analyzed by karyotype analysis and FISH detection. The differences in the detection of abnormal karyotype and abnormal chromosome number are significant . FISH has a higher success rate in the embryo abortion tissue with prolonged necrosis. Effects of ERAS strategy on prevention of complications related to anesthesia in gynecological laparoscopic surgery

Abstract:
Objective To investigate the prevention effects of ERAS strategies on anesthesia-related complications in patients with gynecologic laparoscopic surgery. Methods A total of 156 patients underwent laparoscopic gynecological surgery from October 2015 to October 2016 were included.The patients were randomly divided into control group and ERAS group. The control group was treated with conventional anesthesia. The observation group achieved ERAS strategies to optimize anaesthetic and perioperative management. The VAS scoring at 30 minutes after extubation, rate of PONV, delayed recovery, irritability and postoperative chills were observed. Results ERAS strategies decreased the incidence of PONV and postoperative chills. It shortened the time from PACU admission to extubation and reduced VAS scoring (P<0.05). The differences of PACU residence time, postoperative irritability and recovery delay were not significant (P>0.05). Conclusion ERAS strategies can decrease the incidence of perioperative anesthesia-related complications and promote the early recovery of patients underwent laparoscopic gynecological surgery.

Abstract:
Objective To investigate the expression and clinical significance of CADM1 in the ectopic and eutopic endometrium in endometriosis patients. Methods The expression of CADM1 protein in ectopic and entopic endometrium tissues from 45 patients with EMs and endometrium tissues from 30 patients with normal endometrium from May 2014 to March 2015 were explored by immunohistochemistry. They were detected by Western imprinting method. The hmias-2000 automatic color image analysis system was used to determine the staining intensity and positive rate. Results Differences of CADM1 in endometriosis group, positive expression in tissue and normal tissue were significant (P<0.05). Differences among Western mark CADM1 gray value contrast results were significant (P<0.05). Conclusion CADM1 of endometrial tissue has a expression of downregulation or inactivation in the process of developing the ectopic. Change of CADM1 expression can provide reference for clinical diagnosis and treatment of endometriosis.

Abstract:
In advanced hepatocellular carcinoma, non-surgical treatments such as Transcatheter Arterial Chemoembolization (TACE), radiofrequency ablation (RFA) and other non-surgical treatments may be used when there is no chance of surgery or the patient can't tolerate surgery. TACE and/or RFA are the main method of non-surgical treatments of hepatocellular carcinoma. Accurate diagnosis before or after treatments, early postoperative evaluation of the efficacy of hepatocellular carcinoma are the keys to the greatest benefit of patients after the non-surgical treatments. At present, it is considered that the multi-b value IVIM-DWI technique can not only reflects microscopic changes of tissues,but also reflects the tumor residual or neovascularization before tumor morphological changes. The multi-b value IVIM-DWI technique can evaluate the early effect of treatment for cancer. It is expected to improve the reliability and accuracy of the non-surgical treatments of hepatocellular carcinoma. The paper aims to deepen non-surgical treatment of hepatocellular carcinoma evaluation,and widely promote IVIM-DWI technology in clinical practice.

Abstract:
Subarachnoid hemorrhage is a common disease in neurosurgery with a high mortality. Researches on delayed cerebral vasospasm(CVS) after subarachnoid hemorrhage are explored in China at home and abroad. However, clinical work finds patients with early death after SAH. There is a widespread ischemic injury in the brain that involves the pathophysiological events that occur in the brain tissue prior to late onset of cerebral vasospasm (3 days to 2 weeks).It includs changes in cerebral perfusion, microcirculation, matrix metalloproteinases, digestion of plasminogen activator and other enzymes lead to the loss of type IV collagen in the basilar membrane, distribution in the subarachnoid serum, platelet rich plasma to promote acute vasoconstriction after SAH, platelet activation and aggregation in the microvascular break Cerebral circulation lead to insufficient cerebral perfusion, ischemia and a series of pathophysiological changes. This article reviews the mechanism and progress of EBI after SAH, in order to provide a preliminary theoretical basis for better control of these secondary events and effective treatment of acute SAH in future.

Abstract:
CD11c is a transmembrane glycoprotein mainly distributed on the surface of myeloid cells. Tt is closely related to inflammatory response, immune response and immune cell activation, transformation and proliferation and some other important pathological process. CD11c+cells accumulate in large quantities around the dermal vessels and the lymph nodes.It suggests that CD11c+cells are not simply produced by local cells and are likely to be produced by other cells and transported through the systemic lymphatic-blood circulation to participate in the local inflammatory response. The involvement of CD11c+cells in a variety of inflammatory skin diseases is inextricably related to T cells (Th1/Th2/Th17) and keratinocytes.On one hand, CD11c+cells can participate in the skin inflammatory by affecting the differentiation of T cells.The activation of different phenotype CD11c+DCs is the key to determine the AD to Th2 / Th1 immune response.CD11c+ DCs in psoriatic lesions can induce activated Th1 / Th17 cells to secrete proinflammatory cytokine INF-γ / IL-17.Keratinocytes express human thymus mesenchymal lymphocyte production hormone, which can activate CD11c+ DCs and induce the Th2 immune response.On the other hand, secretion of IL-36 by CD11c +DCs induces keratinocytes to secrete various cytokines and chemokines, further acting on keratinocytes to hyperproliferation and thereby exacerbating dermatitis.

Abstract:
Objective To explore the specific measures for the shortage of nursing human resources caused by the policy of "two-child". Methods The questionnaire was performed on the female nurses in our hospital who aged 20-50 years old. The post with-holding situation due to pregnancy and fertility circumstance in past 2 years were investigated. Results The pregnancy rate in 31-40 years of age was the highest, the differences of pregnancy rate among all age groups were significant (P<0.01). The clinical workload during pregnancy had an effect on the wishes of early termination of maternity leave (P<0.01). Flexible scheduling and certain incentives for nursing staff were effective. Birth year, professional ranks, and their original post affected their wishes of early termination of maternity leave. The growth of clinical nurses was not meet the vacancy caused by pregnancy and childbirth. Conclusion Allocation of human resources can be optimized by establishment of a sound mechanism that reporting wishes of marriage and childbearing, a reasonable planning of recruitment plans, encouragement of nurses, establishing nursing talent pool and mobile nurse library, and the development of male nursing team.It can effectively deal with the lack of post caused by the two-child policy.

Abstract:
Objective To analyze humanized nursing and detail nursing in gynecological disease and explore the satisfaction after treatment. Methods A total of 200 inpatients in our hospital from 2016.7 to 2017.7 were observed. The patients were divided into 2 groups: control group and observation group. Normal nursing was performed in control group and expert nursing was performed in observation group. The effective of nursing and satisfaction between 2 groups was compared. Results Satisfaction of nursing quality in control group was (92±2.17)%. Satisfaction of nursing quality in observation group was (97±4.68)%. The difference was significant (P>0.05). Conclusion Humanized nursing and detail nursing in clinical treatment is better.It improves satisfaction of nursing from patients.
