Correlation between the ultrasonic BI-RADS features and the expression of the Ki-67 in breast cancer

Abstract:
ObjectiveTo investigate the clinical prognosis of the correlation between ultrasonic features described by breast imaging reporting and data system (BI-RADS) and expression of the Ki-67 of breast cancers.MethodsThere were 174 patients hospitalized in breast surgery in Shenzhen Maternity & Child Healthcare Hospital Affiliated to Southern Medical University and 194 lesions of these patients were confirmed by pathology. Their ultrasonic features and expression of the Ki-67 examined by immunohistochemistry were correlation analyzed.ResultsThe sensitivity of ultrasound in diagnosis of breast cancer was 86.1%.The expression rate of the Ki-67 were 63.4% (123/194). Univariate analysis showed angled edge, spiculesign, microcalcifications and rich bloodstream were higher in Ki-67-positive tumors than Ki-67-negative tumors (P < 0.05). Multivariate analysis showed spiculesign and rich bloodstream entered into the regression model (P < 0.05).ConclusionsThere is significant correlation between ultrasound BI-RADS features and the expression of the Ki-67 in breast cancers.According to the ultrasound features to predict prognosis of breast cancers preliminarily will provid some help for clinical treatment.

Abstract:
ObjectiveTo explore the relationship between anti-Mullerian hormone (AMH) and the PCOS hormone and use the ROC curve method to evaluate the diagnosis value of AMH for PCOS.MethodsThe enzyme-linked immunosorbent assay (ELISA) was used to detect the serum level of AMH, the estradiol (E2), follicle-stimulating hormone (FSH), luteinizing hormone (LH), and the testosterone (T) in 503 patients with PCOS, and carried on comparing with 576 women with normal menstrual cycle (control group). Meanwhile, drawh up the ROC curve with SPSS and statistical analyzed these datum.Results(1) The serum hormone levels (AMH 10. 46 ng/ml, E2 40.92, LH 6.67 mU/ml, T 0.78) of PCOS group were obviously higher than those of control group (AMH 3.65 ng /ml, E2 35.97、LH 6.45、T 0.48) (P < 0.05). The FSH level of PCOS group (5.14) was obviously lower than that of control group (6.45)(P < 0.05). (2) Pearson correlation analysis showed positive relationship between serum AMH and LH (r=0.276, P < 0.05) as well as T (r=0.221, P < 0.05). Serum AMH level in PCOS group was nagatively collelated with FSH (r=-0.113, P < 0.05). No significant relationships were found between serum AMH and age, E2 level. (3) The area under the ROC curve for PCOS diagnosed by AMH was 0.926. The standard error of area was 0.008 which has the remarkable significance (P=0.000); while the best dividing value of PCOS diagnosed by AMH was 6.99 ng /ml, the sensitivity was 84%, specificity was 92% and diagnostic efficiency was high. Conclusions Serum AMH level was elevated in patients with PCOS and this may have correlation with hormone imbalance and abnormal follicular development.
MMP-1, MMP-9 and TIMP-1 can be used as an important indicator of the dynamic balance of myocardial matrix remodeling

Abstract:
ObjectiveFrom the homeostasis of MMPs and TIMPs to explore its role in tissue repair and regeneration of cardiac matrix, thus it can provide an effective biological detection methods and treatment of heart failure that caused by matrix remodeling after myocardial injury, myocardial hypertrophy and thickening.MethodsUse Guangzhou Red Cross Hospital (Fourth Affiliated Hospital of Jinan University) for heart patients as research subjects, according to the method of random block cardiac surgery grouped according to different subjects were divided into three groups, the first group of congenital heart disease group (CHD group); second group of rheumatic heart disease group (PHD group); third group of CHD (COR group), because of its atrioventricular size, normal heart function, set it as a control group. Elisa method for preoperative blood mmps and quantitative determination timps seeking dynamic balance between them; intraoperative myocardial tissue cut, shape and distribution of the determination of myocardial collagen matrix by immunohistochemistry; rt-PCR technique myocardial tissue mRNA mmps and timps for testing, to see its mRNA expression.ResultsCHD group, PHD group before surgery blood MMP-3, MMP-9, TIMP-1 levels were higher than COR group, MMP-9 increased even more significant, the difference was statistically significant (P < 0.05), while the CHD group PHD group and no significant difference was not statistically significant; immunohistochemistry showed, CHD group, PHD group, MMP-3, MMP-9, TIMP-1 is widely distributed than COR group, PCR results showed that CHD group, PHD group patients with MMP-1, MMP-9, TIMP-1 mRNA expression increased compared with COR group, the difference was statistically significant (P < 0.05), the same group and PHD CHD group had no significant difference was not statistically significant.ConclusionsMMP-1, MMP-9 and TIMP-1 can be used as an important indicator of the dynamic balance of myocardial matrix remodeling, regulate the activity of MMPs and TIMPs is expected to become the new direction of heart disease treatment.

Abstract:
ObjectiveTo investigate the preliminary application of fast magnetic resonance GRE + Asset sequence dynamic scanning for evaluating swallowing function in nasopharyngeal carcinoma (NPC) patients without pharyngeal symptoms after radiotherapy.Methods80 cases were trained to master the essentials of swallowing, and the fast GRE sequence combined with the Asset technology (acceleration factor are 2.0ph) were sued to perform a single sagittal slice fast dynamic scanning on GE 3.0T MRI system. 60 continuous images of pharyngeal in the slow swallowing state were obtained on each pertain. Dynamic cine images were generated automaticly. All the images were visually evaluated with double blind method.ResultsSuccessful examinations were completed on all of the 80 patients. The pharyngeal morphology were displayed clearly. The clear display rate of the epiglottis, soft palate, tongue and muscles were 92.5%, 98.75%, 93.75%, 95%, respectively. A whole swallowing process of the pharyngeal could be observed clearly on the dynamic cine images, and related changes of the epiglottis, soft palate, tongue, and muscles, which participated the swallowing process, were also displayed perfectly during the swallowing action.ConclusionsThe fast GRE + Asset sequence could be used to evaluate the swallowing function in patients with NPC after radiotherapy.

Abstract:
ObjectiveTo discuss the application of ultrasound elastography imagine in the diagnosis of cervical cancer.MethodsApplication of ultrasound elastography imaging detection on June 6, 2013 -September 2013 clinic in our hospital of 32 cases of cervical cancer patients with lesions and cervical intraepithelial neoplasia (CIN) patients in 5 cases and 20 cases of normal cervix, numerical analysis of statistical record images and measurement.Results32 cases of cervical cancer patients, 5 cases were diagnosed as benign, 5 cases of CIN patients, 2 cases were diagnosed as malignant, there are 3 in 20 normal cervix cases diagnosed as malignant, the sensitivity of ultrasound elastography in the diagnosis of cervical cancer by 84.37%, specific degree 80%, 82.45% accuracy.ConclusionUltrasound elastography imagine in the diagnosis of cervical cancer has higher application value and clinical significance, but the ultrasound elastography imagine in the score of precancerous lesions and cervical cancer crossed, because of the limitation of sample size should be larger sample size for validation. Ultrasound elasticity imaging at the same time is expected to be used for cerical cancer chemotherapy effect evaluation and provide objective basis in clinical stage.

Abstract:
ObjectiveTo discuss the effects of different anesthesia on the cellular immune function of perioperative period in patients with laparoscopic radical prostatectomy.Methods18 patients with laparoscopic radical prostatectomy were randomly divided into two groups: total intravenous anesthesia group (groupA) and general anesthesia reunite with combined spinal-epidural anesthesia group (groupB), 9 cases in each group.Venous blood were sampled respectively in 1h before anesthesia (T0), 1h (T1), 24 h (T2) and 7 d (T3) after anesthesia, then determined between the two groups at each time period of T lymphocyte subsets (CD3+, CD4+, CD8+, CD4+/CD8+ratio) and NK cell (CD16+56+) levels, and then analysis the changes of the indexs between different time points after operation and T0 in two groups.ResultsTwo groups of patients were decreased in the levels of T cell subsets (CD3 +, CD4 +, CD4 +/CD8 +ratio) and NK cells (CD16 +56 +) at T1 compared with T0, and the levels of group A decreased significantly (P < 0.05). Two groups of the levels of T cell subsets (CD3+, CD4+, CD4+/CD8+ratio) and NK cells (CD16 +56 +) decreased lower at T2 than T0 (groupA:P < 0.01, groupB:P < 0.05), and group A decreased lower than group B (P < 0.05). All the datas of the two groups were closed to the levels of T0 at T3 (P>0.05). The levels of CD8+ changed unsignificantly at all time points in two groups (P>0.05).ConclusionsBoth total intravenous anesthesia and general anesthesia reunite with combined spinal-epidural anesthesia have different degrees of temporary inhibition of cellular immune function for patients with laparoscopic radical prostatectomy, the latter is a lesser inhibitory effect.

Abstract:
ObjectiveTo study the patients who had primary trigeminal neuralgia using CT and MRI in thin slicer scan, in order to find a dependable new target to treat primary trigeminal neuralgia used by CT positioning for radiotherapy.Methods95 patients got 17Gy extend to 29Gy doses by χ-ray knife using the trigeminal impression on the crista. We had studied the 50 cases thin CT slice who had primary trigeminal neuralgia, CT slice thickness is 1.25 mm and 0.625 mm, to research MRI and CT image fusion.ResultsThe rate of impression was 94% for 1.25mm CT slice thickness and 100% for 0.625mm respectively, finding the impression on the crista of petrosal bone in compared with the trigeminal position was identical completely, accurate rate was 100% and effective rate was 85.26% for radiotherapy.ConclusionsThe trigeminal impression on the crista of petrosal bone was a dependable symbol for the radiotherapy of primary trigeminal neuralgia patients.

ObjectiveTo analyze and study the treatment of subcutaneous cystic fluid useing lumbar drainage in syringomyelia postoperative.MethodsRetrospective analysis 28 cases of subcutaneous cystic fluid of syringomyelia postoperative in patients since July 2010 to July 2014 in our hospital, including 19 cases of patients useing lumbar drainage and occipital bandaged treatment, nine cases of patients using multiple lumbar puncture and occipital bandaged treatment.ResultsThe disappearance of subcutaneous cystic fluid in lumbar drainage and occipital bandaged therapy group was significantly shorter than the multiple lumbar puncture and occipital bandaged group (P < 0.05), comparing the difference between the two groups was not statistically significant in complications (P>0.05).ConclusionThe clinical efficacy of lumbar drainage and occipital bandaged treatment of subcutaneous fluid after surgery of syringomyelia is better than traditional lumbar puncture and occipital bandaged, is a simple, less invasive, fewer complications methods, and avoiding secondary damage caused by surgical repair dura.

Abstract:
ObjectiveTo investigate the effect of enhanced antihypertensive therapy on the hospitalization time, the efficacy and safety of the patients with chronic heart failure with hypertension.Methods64 patients with chronic heart failure with hypertension from March 2010 to December 2014 were randomly divided into observation group and control group with 32 cases and 32 cases. The control group (standard antihypertensive group) was treated with conventional therapy alone, and the observation group (enhanced blood pressure group) was treated with nitroglycerin needle for the treatment of conventional therapy. To observe and compare the effect of the two groups,Resultsobservation group 32 cases, 15 cases markedly effective, effective in 15 cases, invalid in 2 cases, no deaths. The total effective rate was 93.75%; control group 32 cases, 7 cases markedly effective, effective in 14 cases, ineffective in 10 cases, 1 case died, the total efficiency is 65.63%. The total effective rate of the two groups was significant (χ2=7.82, P < 0.01). In the observation group, there were 2 cases ofχ hypotension, and the symptoms disappeared after treatment.ConclusionIt is a safe and effective method to improve the treatment effect and shorten the hospitalization time for patients with enhanced hypertension.

ObjectiveTo compare the CT scanning and χ ray in measuring the volume of the effusion for the cavity caused by pulmonary tuberculosis or lung carcinoma.MethodsWe collected simultaneous χ-ray sternum and breast enhancement CT scan confirmed empty and effusion of the 36 and 32 patients with tuberculosis and lung cancer patients, both in CT guiding effusion puncture, compare the three methods of measuring the amount of effusion.ResultsThe tuberculosis hole and cancer patients were measured by enhanced CT chest effusion quantity more obvious than χ-ray plain film (P < 0.05), and piercing actual amount drawn effusion more equally than χ-ray plain film (P < 0.05), but the chest CT and the quantity measured effusion puncture income no statistical difference.ConclusionFor patients with the same point at the same time, the chest CT enhanced scan cavity effusion quantity closer to actual measured by volume, prompt CT measurement precision was higher.

Abstract:
ObjectiveTo summarize the application experience of double pulse extracorporeal shock wave lithotripsy (ESWL) in the treatment of residual kidney calculus after PCNL operation.MethodsThe clinical data of 3265 kidney calculus patients underwent ESWL were analyzed retrospectively. The patients of residual kidney calculus after mini-PCNL as experimental group (n=896) and the patients who underwent simply ESWL as control group (n=2369). The effective rate, cure rate and complications were compared.ResultsThe proportion of patients in the experimental group than ESWL treatment for secondary low ESWL treatment group compared to the simple, high stone expulsion rate than pure ESWL treatment groups. In terms of complications, postoperative occurrence of gross hematuria, postoperative infection, under the kidney capsule or renal parenchymal hematoma considerable proportion of dysfunction. However, the experimental group after the treatment of patients with renal colic and proportion than the pure form of Stone Street ESWL treatment group is low.ConclusionThe double pulse extracorporeal shock wave lithotripsy for the treatment of patients with residual stones after PCNL less invasive, high success rate, less serious complications, is one of the effective treatment of upper urinary tract calculus.

ObjectiveTo study the intrauterine growth retardation in fetal cord blood Ghrelin, SP1 expression levels, explore their relationships with intrauterine growth retardation.Methods30 cases of intrauterine fetal growth retardation, 30 cases of normal fetuses, using enzyme-linked immunosorbent assay were used to detect the normal newborn and intrauterine growth retardation and neonatal cord blood Ghrelin levels SP1.ResultsIn the observation group, newborns, birth weight and umbilical cord blood Ghrelin levels (r=-0.215, P=0.041) was negatively correlated with cord blood SP1 level (r=0.376, P=0.035) were positively correlated. Relations in the control group newborns, birth weight and umbilical cord blood Ghrelin levels (r=-0.218, P=0.097), with the SP1 level (r=-0.225, P=0.099) is not obvious. Observation group compared with the control group, Ghrelin levels higher than normal control group, SP1 level lower than the control group, the difference was statistically significant.ConclusionThe high levels of Ghrelin, low levels of SP1 might cause intrauterine growth retardation.

Abstract:
Endotoxemia is one of the most common systemic inflammatory response syndrome and triggered by Gram-negative organisms, which can release endotoxin. Myocardial injury during endotoxemia is one of the leading causes of patient death, however, the underlying mechanism is not clear. More and more studies are focusing on the mechanism of myocardial injury during endotoxemia. These studies are mainly focus on regulation of inflammatory factors, myocardial energy metabolism, myocardial apoptosis and microvascular dysfunction. In this paper, we summarize the current progress of mechanism of myocardial injury during endotoxemia, providing new direction for the future reaserch and theoretical basis for the treatment.

Abstract:
Lung cancer have the highest mortality rate in all malignant tumors currently. Also, its five-year survival rate is very low. Although the chemotherapy and operation level has been greatly improved, the mortality of lung cancer remains unchanged. It is very important that prevent, anticipate, and diagnose the cancer as soon as possible. MicroRNAs (miRNAs) are small non-coding RNA molecules capable of regulating expression of over 50% of protein-coding genes. The RNA molecules are stable in tissues and blood, so it can tend to be a biomarker in early diagnosis of lung cancer. Here, this is a review on the roles of miRNAs for possible ways to prevent lung cancer in clinical trials. With the improving understanding of the pathogenesis in lung cancer at the molecular, especially the study on MicroRNA theory, brings new hope for the early diagnosis and treatment of lung cancer.

Abstract:
ObjectiveTo probe into the case of abdominal necrotizing fasciitis caused by Escherichia-coli-infected appendicitis.MethodsRetrospectively study and comprehensively sum up the analysis of medical imaging, histology, cytology and pathology of appendix abdominal necrotizing fasciitis, which is a case caused by colon cancer invasion and Escherichia coli infection.ResultsCT scan of the case shows abdominal diffuse edema, extensive subcutaneous pneumatosis, psoas abscess, ring-shaped enhancement after strengthening; irregular cecum wall thickening, appendix wrapped around the cecum, thickening and shortening of appendix distal, obvious enhancement after strengthening, periappendiceal inflammatory infiltration. Bacteria cultured from abdominal wall fester are Escherichia coli, which is confirmed to be cecum adenocarcinoma by endoscopy and pathological examination. CT scan also shows appendiceal orifice occlusion. After effective clinical treatment and recheck, there is significant reduction of abdominal wall abscess and subcutaneous pneumatosis.ConclusionFor abdominal necrotizing fasciitis caused by Escherichia-coli-infected appendicitis, early diagnosis should be made according to abdominal necrotizing fasciitis symptoms, signs and characteristics. Proper examination methods should be adopted to understand the extent of lesion and the pathogeny so as to provide timely and prepared information for clinical treatment.

Abstract:
Orthopedic clinical physical examination has been one of the key elements in the teaching of all orthopedic physical examination in the diagnosis of bone disease in a pivotal position. The orthopedic surgeon should and must first patients with a comprehensive physical examination as the important information for diagnosis. Standard physical examination skills training to strengthen the teaching of orthopedics in the new situation and improve the accuracy of physical examination results have important scientific and clinic significance.

Abstract:
Clinically nursing errors happen very often. To correct errors caused by carelessness of nurses, a nursing information system based on internet of things was introduced in this paper. This system was built and tested in a clinical section and satisfaction survey of nurses was used to evaluate its performance. Results showed that nurses'workload can be dynamically reduced and most nursing errors can be corrected by our system, which proved its importance in nursing information.

Abstract:
ObjectiveTo seek a more reasonable way of traditional Chinese medicine dispensing, meet the demand of patients.MethodsCarries on the questionnaire survey to come round to see a doctor of traditional Chinese medicine clinic patients, combined with the practice of hospital, from the safety, effectiveness, economy, simplicity and education five aspects analysis of bulk pieces of Chinese medicine, TCM formula granules and small package of Chinese herbal pieces.ResultsBulk pieces of Chinese medicine maintain a safe, effective and economical characteristics, also is advantageous to the cultural heritage of traditional Chinese medicine, but quality is not stable, adjust the problem of inaccurate; TCM formula granules is simple, more adapt to the rhythm of modern life, but because of lack of efficacy is not to determine the safety and efficacy, is not conducive to the cultural heritage of Chinese medicine; Small package of Chinese herbal pieces maintained the characteristics of traditional Chinese medicine, safety, efficacy guaranteed, but there are high prices, convenient to carry, unpacking complicated issues.ConclusionThree adjustment methods have pros and cons, should avoid weaknesses, complement each other, promoting the sound and rapid development of the cause of the development of traditional Chinese medicine.

Abstract:
ObjectiveTo investigate and research measures to strengthen psychological care of children with precocious puberty application results.Methods100 cases of patients selected as precocious children between 2008.1~2014.1 our hospital, the use of randomized divided into two groups, a control group taking only routine clinical care measures, the observation group in the control group on the basis of intensive psychological care, treatment Triptorelin two groups of children in the same manner, using the Child Behavior Checklist for Ehrlich explicit behavior in children before and after treatment were evaluated.ResultsThe two groups of children with varying degrees compared to pre-treatment improved overall foreign children with significant behavioral problems observed group scores were significantly lower than the control group, the difference was statistically significant (P < 0.05).ConclusionStrengthen psychological care measures can effectively improve the psychological condition of precocious puberty in children, reduce behavioral problems in children with explicit, with the exact efficacy in the treatment of precocious puberty in children, it is worth to be in clinical practice.

Abstract:
ObjectiveTo investigate the effects of medical and nurse quality and ward-related satisfaction under the cooperative responsibility nursing model.MethodsTo select the patients in the department of gynaecology in our hospital between Jan 2014 to Jun 2014 as the control group (before undergoing medical and nurse cooperative responsibility nursing model), the patients between Jul 2014 to Dec 2014 as experimental group. The control group carred out the traditional nursing rounds mode, the experimental group performed medical and nurse cooperative responsibility nursing model, to compare the medical and nurse quality and ward-related satisfaction between the two groups.ResultsThe days of hospitalization, medical grade medical record rate, patient satisfaction were better than before the implementation of medical and nurse cooperative responsibility nursing model (P < 0.01).ConclusionThe medical and nurse cooperative responsibility nursing model cooperative medical improves the health care accountability patient communication, increases the work efficiency, it also does benefit in improving the quality of medical care, helping deepen quality care, and strengthening specialist care connotation.
