Advances in research on loss of adjacent contact after posterior dental implant crown and bridge repair

摘要: 后牙种植冠桥与天然邻牙之间的邻接触丧失是种植义齿修复后最常见的并发症之一。天然邻牙可发生近中漂移,而种植体无此生理学现象被认为是种植义齿邻接触丧失的主要原因。此外,对颌牙类型、上下颌骨骨密度差异、缺牙修复时间、游离端修复、邻牙骨支持水平等因素均参与引起种植义齿邻接触丧失,而后者可直接造成食物嵌塞、邻牙龋坏、牙周炎种植体周围炎等不良后果。本文从种植义齿修复后邻接触丧失的发生率、种植牙的生物学特性、参与邻接触丧失的因素、邻接触丧失的不良后果及其解决方案等5个方面,对后牙种植冠桥修复后邻接触丧失进行综述。Abstract: Recently, proximal contact loss between implant-fixed prostheses has been reported as one of the most frequent complications in dental implant restorations, which has a high prevalence. Mesial drifting of adjacent natural teeth was presented as a main cause of PCL at the mesial site of the implant which has no physiologic phenomenon such as mesial migration . Besides, there are other various associated factors could be related, concluding the opposing dentition, undergo short time interval of tooth loss before restoration, free-end fixed implant prostheses, the occlusion of adjacent teethand the alveolar bone support level of the adjacent teeth and so on. The proximal contact loss can have negative effects on the dentition: food impaction, caries, periodontal issues, and peri-implantitis. So proximal contact loss has been aroused wide public concern. This article reviews the prevalence, the potential causes, clinical significance, and treatment of open contacts between dental implant restorations and adjacent natural teeth.
